Other:**We are currently putting all brace & balance applications on hold until further notice. You can also find other service dog organizations that may provide brace & balance service dogs through Assistance Dogs International (www.assistancedogsinternational.org). We do not provide service dogs for the following: • Seizure detection dogs• Allergy detection dogs• Blood sugar detection• Hearing dogs• Guide dogs• Emotional support dogs• Psychiatric service dogs To start the process clients must submit a complete application. The application has four parts; the client portion, the medical form (to be completed by their doctor), the video/photo outline, and two letters of reference. There is a $25 application fee that should accompany all applications. Interviews will be scheduled only when all portions of the application have been received.
