Purple Heart:Purple Heart Recipients or 30% rating or above requesting any kind of assistance, fill out application form
Other:Submit a copy/scan of the documents you have recieved from the Department of Defense or the Veterans Administration (PEB or service connected rating document) . If you have been discharged from service please provide your DD214 as well. If you are still on active duty, a scanned copy of your Military ID will suffice.
